慢性牙周炎患者血清CTRP3、CYTL-1水平及其临床意义

摘要:

目的:探讨血清C1q/TNF相关蛋白3(CTRP3)、细胞因子样蛋白1(CYTL-1)在慢性牙周炎患者中的表达及其与牙周指标和炎症指标的相关性。方法:选取于2022年4月至2023年4月收治的慢性牙周炎患者90例列为牙周炎组,依据牙周情况分为轻度组(33例)、中重度组(57例),另选取同期至本院体检口腔健康者40例列为健康组。探诊牙周炎组的牙周指标:探诊深度(PD)、出血指数(BI)、菌斑指数(PLI)、附着丧失(AL)、牙龈指数(GI);ELISA法检测血清指标:白细胞介素-1 β(IL-1 β)、干扰素-γ(IFN-γ)、肿瘤坏死因子-α(TNF-α)、CTRP3、CYTL-1。分析血清CTRP3、CYTL-1与炎症指标、牙周指标的相关性;比较CTRP3、CYTL-1诊断慢性牙周炎严重程度的价值。结果:牙周炎组的IL-1 β、IFN-γ、TNF-α高于健康组,CTRP3、CYTL-1低于健康组(P<0.05);中重度组的IL-1 β、IFN-γ、TNF-α、PD、BI、PLI、AL、GI高于轻度组,CTRP3、CYTL-1低于轻度组(P <0.05);CTRP3、CYTL-1与IL-1 β、IFN-γ、TNF-α、PD、BI、PLI、AL、GI呈负相关(P <0.05);血清CTRP3、CYTL-1联合诊断曲线下面积(AUC)优于两者单独诊断AUC(P<0.05)。结论:慢性牙周炎患者血清CTRP3、CYTL-1水平较低,与牙周指标和炎症指标有关,可用于诊断慢性牙周炎的严重程度,联合检测更具诊断价值。

Abstract:

AIM:To investigate serum levels of C1q/TNF-related protein 3(CTRP3) and cytokine receptor like factor 1(CYTL-1) in patients with chronic periodontitis,and to assess their correlation with periodontal and inflammatory indicators.METHODS:90 patients with chronic periodontitis admitted to our hospital were enrolled as the periodontitis group.Based on the periodontal severity,they were classified into a mild group(33 cases) and a moderate to severe group(57 cases).Another 40 oral health patients who underwent physical examinations at our hospital during the same period were as the healthy control group.The periodontal indicators-including probing depth(PD),bleeding index,plaque index(PLI),attachment loss(AL),and gingival index-were recorded.Serum levels of Interleukin-1β,Interferon-γ(IFN-γ),Tumor necrosis Factor-α(TNF-α),CTRP3,and CYTL-1 were measured by enzyme-linked immunosorbent assay.The correlation between serum CTRP3、CYTL-1 and gingival crevicular fluid and periodontal indicators was analyzed;the value of CTRP3 and CYTL-1 in diagnosing the severity of chronic periodontitis was compared.RESULTS:Compared with the healthy control group,patients with chronic periodontitis had significantly higher serum levels of IL-1 β,IFN-γ,and TNF-α,and significantly lower levels of CTRP3 and CYTL-1(P<0.05).The moderate-to-severe group exhibited higher levels of inflammatory markers and worse periodontal parameters than the mild group(P<0.05),accompanied by lower CTRP3 and CYTL-1 concentrations(P<0.05).Serum CTRP3 and CYTL-1 levels were negatively correlated with IL-1 β,IFN-γ,TNF-α,PD,BI,PLI,AL,and GI(P<0.05).Combined detection of CTRP3 and CYTL-1 yielded a significantly greater area under the ROC curve(AUC) for diagnosing disease severity compared to either marker alone(P<0.05).CONCLUSION:The serum levels of CTRP3 and CYTL-1 are significantly decreased in patients with chronic periodontitis,and are related to periodontal status and systemic inflammation.They can be used to diagnose the severity of chronic periodontitis,and combined detection has more diagnostic value.
